This form should only be used to add visa pages to a currently valid U.S. passport.
Although the form indicates that persons residing abroad may not use this form by mail to the indicated U.S. address, this restriction does not apply to resident of Canada. Complete the form and follow other instructions as indicated.
There is no charge for routine mail service, which is receipt of your passport with added pages, within 4 weeks from the date you mail it.
Important: Applications which are incomplete, or which make use of the wrong form, or which are directed to the wrong office, will be returned to the applicant for correction. You may apply by mail or in person at the U.S. Embassy/Consulate. Enclose the following:
- Completed and signed Application for Additional Visa Pages DS-4085, which form is also available at the nearest U.S. Embassy/Consulate.
- Your valid U.S. Passport.
If you need your passport back in less than 2 weeks for international travel or for foreign visas, you should submit Form DS-4085 in person. The passport is normally returned to you by regular first class mail. If you wish to expedite or secure the return of your passport, please provide a pre-paid regional Express Post or Priority Post Courier envelope.
