Each applicant must bring a completed and signed DS-156 to the interview. Click here for detailed instructions about the DS-156. You may click the button below to fill out Form DS-156 online. You must then print the resulting completed document, including the barcode on the last page which contains all of the information you have entered. This will help the U.S. Consulate to process your application more quickly.
Form DS-156 can be filled out on-line to assist in the processing of your application. If you have not updated your browser in a long time then it is possible that you may encounter some difficulty using the online e-form. Most users will have no difficulty.
If your form submission results in an error message then please try again later. Once in a while, the server becomes overwhelmed by there being too many users at the same time.
All visa applicants must submit electronically-completed DS-156 application forms. Hand-written DS-156 forms will not be accepted. Forms must be completed before coming to the Embassy.
You MUST complete the electronic form on-line (all 3 pages). Print out the entire form, including the third (barcode) page, and bring it with you to the interview. Applications without a complete barcode will not be accepted. For more information and examples, please review this document.
